Collection and Use of Personal Information
The Pharmox may collect personal information from a user of the Website (the “Site User”) when the Site User uses the Pharmox website. The Pharmox may use such personal information as is reasonably required to process the Site User’s registration for the Website and/or to provide information and other features on the website that may be useful to the Site User.
Contact information that the Site User provides to the Website may be used to contact him or her in connection with the Website. For instance, depending on which services the Site User has subscribed to and which privacy preferences the Site User has indicated to the Pharmox, the Pharmox may use the Site User’s contact information to provide notices regarding changes to the Website or regarding new or updates to the services or products.
The Pharmox may collect and analyze non-personal information to evaluate how visitors use the Website, including personal information about usage and service operation that is not associated with a specific personal identity. The Pharmox also automatically receives and records information on the Pharmox’s server logs, which information may include the Site User’s computer’s IP address and “cookie” information. The Pharmox uses the Site User’s IP address to help identify the Site User, gather broad demographic information about the Pharmox’s users, diagnose problems with the Pharmox’s systems, and administer the Website. A cookie is a small text file that is stored on the Site User’s computer and cannot read data off of the Site User’s computer. The only information a cookie can collect is information supplied by the Site User.
The Pharmox collects demographic and profile data in connection with the Website and uses such data to tailor each Site User's experience at the Website and displaying the content according to the Site User’s preferences.
The Pharmox may share personal information with third-party subcontractors and agents who work on the Pharmox’s behalf and third party service providers to who provide advice and services in relation to the website or in relation to products or services featured on the website. Such third parties do not have the right to use personal information beyond what is necessary to assist the Pharmox and they are contractually obliged to maintain confidentiality and security of the personal information and are restricted from using such information in any way not expressly authorized by the Pharmox.

The Pharmox may disclose personal information as follows:
to third party service providers and to affiliated entities in order to carry out work on behalf of the Pharmox in relation to the website,
to law enforcement agencies for the purposes of investigating fraud or other offences,
to legal, financial, insurance, and other advisors or in connection with the sale, reorganization, or management of all or part of its business or operations, and
as consented to by the Site User from time to time, including to fulfill any other purposes that are identified when the personal information is collected.
The Pharmox will not use or disclose personal information for purposes other than those for which it was collected, except with the Site User’s consent or as required or permitted by law. Consent may be express or implied, and given in writing by using or not using a check-off box, electronically, orally, or by the Site User’s conduct, such as accessing the Website and/or use of the services or products provided by the Website.
Storage of Personal Information
While in another jurisdiction for processing, the information may be accessed by the courts, law enforcement, and national security authorities.
Cookies and IP Addresses
The Pharmox protects against the loss, misuse, and alteration of personal information with security measures appropriate to the sensitivity of the information, including through the use of physical, organizational, and technological measures and appropriate training of employees. However, it is possible that any information transmitted via the Internet may be intercepted by unknown third parties. The Pharmox is not responsible for the transmission of personal information that is disclosed or becomes transmitted through no omission or act of the Pharmox.
When appropriate, Site Users are given the opportunity to (i) opt-out of receiving communications from the Pharmox, (ii) remove their personal information from the Pharmox’s records, or (iii) elect to no longer receive services from the Pharmox. Terms & Condition
The standard shipping policy for Pharmox is as follows:
Orders received prior to 4:30PM EST*, Monday through Friday, will be processed and shipped the same day in most cases. Orders placed after our cut off times will be shipped the following business day. Orders placed on Saturday, Sunday and company-observed holidays will be shipped the next normal business day.
Pharmox's order minimum is $125.00. All minimum orders are shipped freight pre-paid at company expense. Refrigerated products Thursday through Sunday will be shipped the following Monday. Pharmox reserves the right to change or modify this policy at any time and may make exceptions as deemed necessary.
*Excluding orders held for Credit or Compliance Review.
By returning merchandise, you are certifying that the specific product(s) were purchased directly from Pharmox ; that product(s) have been stored properly at a temperature indicated on the product label and in the original bottle; bottles have never been opened and drug product(s) have never been repackaged, in compliance with the Prescription Drug Marketing Act.
Returns are only accepted on purchases made directly from Pharmox.
Returns are only accepted from the original purchaser.
Credit will be based on the current selling price or original invoice price, whichever is lower, adjusted for terms discounts, rebates, free goods, etc. if applicable.
Credit will be issued within 30 days of receipt. No deductions may be taken prior to that time. All credits will be issued in the form of a credit memo, no checks will be mailed.
Sales representatives cannot authorize return of products or pick up merchandise. All returns must be authorized by a manager.
• NO CREDIT will be given for the following:
Any product purchased as short-dated or close-out inventory.
Products shipped back to Pharmox with lot numbers other than what the customer has received from Pharmox Pharmaceutical.
Opened or partial bottles; Repackaged products or products not in original sealed container.
Packages that arrive at our distribution center without a return authorization form.
Merchandise sent to our distribution center that has not received prior authorization. Such items will not be returned and will be destroyed.
Product defaced with stickers, price marks, knife cuts, etc.
Damaged merchandise due to improper storage and handling, re, ood, heat, cold, water, smoke, catastrophe or bankruptcy sale, or purchased or otherwise obtained in violation of any Federal, State or local law or regulation.
Merchandise returned by a customer that has not made a purchase for a period of two months or longer.
Freight unless the return is a result of our error.

Shipping errors must be reported within 48 hours of receipt and are returnable for full value at our freight expense.
Pharmox will provide a call tag for returns due to our error. Please note these tags are only valid for 10 days and may only be used for products shipped in error.

You agree to defend, indemnify, and hold harmless Pharmox LLC and its affiliates, employees, partners, and officers from any claim made by a third party arising from and against any and all actions, causes of action, liabilities, claims, suits, judgments, liens, awards, and damages, or any kind or nature whatsoever, related to your use of this Website, including without limitation all costs, expenses, and attorneys’ fees, including those related to the establishment of this indemnification, whether or not specifically awardable under court rules.
Payment of Fees
When you elect to enter a form of payment, you agree that: (i) you will fulfill your obligation to pay for the product or service by the date on which payment is due; (ii) any payment information you provide is true and accurate; (iii) you are authorized to use the payment method you provide; (iv) we have permission to retain the payment information and method submitted by you and your issuing bank or the applicable payment network; and (v) we are authorized to charge you for the product or service using the established payment method and the information you provide that is stored in your account as of the applicable date for payment. Your assurances of payment and our rights to collect payment include any obligation you may have to pay any taxes or delivery charges described as being applicable to the product or service you are buying. In addition, you agree that you will be responsible for any credit card chargeback fees as well as any reasonable collection costs we incur as a result of your failure to pay on a timely basis.

TERMS AND CONDITIONS: This application is for direct ordering of pharmaceutical products and is subject to acceptance of Pharmox LLC. The terms and conditions, including pricing, related to sale of products are fully set forth in the account price list and are subject to change at any time. It is the responsibility of the account holder or its representative to report any discrepancies of any matter. If this application is accepted, the newly approved account holder and its representatives agree to be bound by the terms and conditions prevailing at the time of each order. Should there be any conflicts between the terms and conditions of the prevailing pricing list, the former terms and conditions shall prevail. Pharmox will follow Industry guidelines concerning “suspicious orders” and will develop thresholds of items ordered based on quantities specified on this application. At any point Pharmox retains the right to ask specific questions pertaining to customers reaching and/or exceeding thresholds of ordering controlled substances. Pharmox reserves the right to hold any order out of compliance based on this application. Background checks may be performed on all persons listed on application. Customer must contact Pharmox before conducting prescription internet fulfillment.
DISCLAIMERS: Pharmox reserves the right to amend this policy. This policy supersedes all previous policies and is hereby automatically incorporated into any prior customer supply agreements or written contracts.
